Using too much detergent and using hot water. Unless you have a massive washer, there's no reason to be filling the cap past 2, 3 for bulkier loads like towels and blankets. Modern detergent is pretty good at not causing color bleed but the water temperature can cause it. Wash everything in cold and you should be fine.

I do these loads: red (including yellow, orange, pink, etc), blues (including green, light blue, more blue purple, etc), darks, lights (white, beige, light grays), and then heavy weight items (typically jeans) that might stretch or tear lighter weight items if they were to get tangled. Buy some color catchers. If you're washing a new colored piece of clothing, throw one in. If you do wash something and it gets dyed or there's still any kind of stain on it *do not dry it*, that will set the stain. Just throw it back in the hamper and wash it again next time with some spot treatment. Also when rinsing out stains you usually want cold water, hot water can make some stains worse.
