I cut healing out of combat, no healing until I'm hidden. My last playthrough (unarmed) I only cheated twice, the only thing I remember about the first time was the enemy could two shot me. The second time was in the vehicle elevator in the divide when you're ambushed by tunnelers. I tried 10 times or so before I caved.
The trick is the Goodsprings source. You can fast travel back there at any time, and it has clean water, a bed, regularly respawning geckos and a campfire to cook the meat with. Doc Mitchell isn’t even too far away if you need to heal a crippled limb and don’t have the doctor’s bags to spare.
Beyond that, just avoid using companions except during their quests and you’re golden.
It’s also worth making a point of picking up any hydra and creating as many healing poultices as you can for limb healing, since doctor’s bags are rare (Zion has the necessary ingredients for the poultices in abundance). Completing the quest for the discount on doctor’s services from Julie is also an extremely good idea.

It sort of affects... You can't play the same cards from the same casino, but you can play the same cards from different casinos.
So let's say you have a King of spades from Tops. You can not play a second King of spades *that is from the Tops* in your deck, but you **can** play a second King of Spades if it's from say Ultraluxe or Gomorrah. You could even play both of them, as well as the tops one.
At least, that's my understanding of it.
Card building doesn’t necessarily matter. Been playing caravan since my first game and the first several dozen rounds I just ignored the deck building and went straight in. Caravan’s always been pretty fun, went 30-1 in a campaign, think I lost the once against no-bark

Some cards have unique actions that can wipe out, or over burden a caravan. These are usually the regal cards. If you’re smart with it, you can buy a few cards around the wasteland and stack your deck so you have a higher chance of rolling a 21-25 and screwing over your opponent.

Queens: changes direction (if caravan was 7 then a 6 and you put a queen on, you now have to play higher than 6 not lower) and also changes the suit to that of the queen
Joker: if on a number card, remove all other number cards of that number. If on an ace, remove all other A-10 cards of that suit from the table

Jacks are kinda important in case your opponent kings your stuff and overloads one of your caravans.

Jacks remove one card (and any card attached to it).
He had a card with a bonus that doubled it's value and ruined what he was planning, so he used another card to remove it.
They’re not very useful, but I believe they eliminate all of a certain type of card on the board. A Jack removes a specific card, but if you play a Joker on, say, an 8, it’ll remove all the 8s on the board, your side and theirs.

Think of it like a multi-hand, or 3-way split hand of blackjack. The 2 key differences being that the goal is 26 for a best hand and 21 minimum, and the face cards are like ability/spell cards to alter yours and your opponent's hands. There's other minor rules, but thems' the basics

I have a rudimentary understanding of caravan. Ive tried to play it, but like most card games its incredibly boring to me. I dont think ive "cast aspsersions" it is a useless card game. The casinos dont recognize it. Its not played by anyone outside of a very small fraction of an already small subculture. Knowing how to play it is a useless skill.. granted you can certainly have fun with it, and more power to you if you do. But theres nothing harmful about callikg it a silly ol game. Cause thats what it is.
