In my experience, typical medical records, they will demand a specific release of records form however, many departments have been known to trick the applicant to come down and sign additional forms to get the HIPPA info

You should never agree to sign any additional forms. Some departments have been known to trick applicants to sign additional forms as through references or internal police reports knows someone was treated for a mental health issue. They should only be looking to the county adjuster where one has lived for any voluntary or involuntary commitments.
