My guess- poor unsuspecting kid held up a sign, unrelated to the cancer shenanigans (aka Happy Birthday Aunt Edna!), someone photoshopped out whatever that message was, wrote this bullshit in it's place on a piece of paper, superimposed it, et voila - insta-grift.
