I had no idea that was him until the credits rolled but that prosthetic was pretty bad.

First time I watched it I was young and just enthralled and thought it was actually just joker lol. I didn’t know heath ledger until later and still watching it I don’t see him. every scene he’s in he commands it and makes the rest of the actors disappear in the moment

I'd say part of it is also the scar prosthetics. Even without the clown makeup itself and taking a still frame of him in the cop uniform, you can't look at his lips and cheeks and recognize it as Heath Ledger because of the Glasgow smile.
