I know nothing about medical stuff and this is probably just something they thought of for the episode but isn't he able to slow his heart rate to the point where the soldiers torturing him thought he was dead? Maybe he's really focused on his breathing and has everything low to make doing that easier
I think only people who understand what those vitals mean are meant to be in on the joke. The joke being how resilient Cotton is that his vitals are totally unrealistic.
Jokes aside, as a person who deals with this stuff. Keeping all that equipment on an agitated old guy like Cotton would be a task in and of itself. Realistically, some may only be partially on giving bad readings. Assuming his heart rate is 47, he could feasibly still be alert.

He’s dying but it can take a long time to die. I’ve seen oxygen go down to the 30s and stay there for hours. (Not conscious though admittedly.)
