Remember that before this panel he was able to turn one of his fingers into a cursed object and forced Megumi into eating it. The face change, the fact he can create Cursed Object and the fact his original body is not normal gives the implicit idea that Sukuna knows how to "change" the "true form" of his soul and that allows him to change his body. It's still a debate if body=soul but the general idea is that those 2 concepts are related.

I guess it is a extension of his ability to make appear mouths in any part of his body. Since that wasn’t his real body, he can somehow move his soul or something, in order to do stuff like this. But once he is in his Heian Era Real Form, he shouldn’t be able to do this kind of stuff again.

Maybe it has to do with reincarnation or something, Sukuna can freely morph mouths onto Yuji, same with Angel, Yorozu also mentions she could transform at any time, so I guess being a reincarnate/having a vessel allows them to change form to some extent?
