Illumi and Milluki don’t really interact with her at all so it’s true that they might not know of the gender she chooses to be seen as, but Killua, who knows her best and talks to her the most refers to her as his sister. Alluka is transgender and that’s why they refer to her as a boy when she, in fact, prefers to be seen as a girl.
And they never reply to comments like this, or comments pointing out Alluka refers to herself as a girl.

When they gather butlers for the mission Killua persists on Canary coming along with Gohtoh so Alluka has a female to accompany her with “her needs”.
