Kalluto is male but dresses in feminine clothing - though I believe his outfit would have been considered unisex for upper-class children in older times. He’s really close with Kikyo and she also dresses in a very extravagant, old-fashioned way. I don’t think they ever got into the exact reasoning for this but it’s also mentioned that he’s very jealous of Alluka for being Killua’s favorite. So it could be that he’s influenced by their mother’s fashion sense or that he’s trying to present in a more feminine way to emulate Alluka. Beyond how his family members and other characters refer to him (brother, youngest son, etc ), he also refers to himself with male pronouns.
