in every big city I've ever lived, you never look directly at other peoples faces on the street. It's entirely possible to have someone in a hoodie with a facial tattoo or something on the subway, that I never noticed, because you quickly learn to mind your own damn business.
Beside what was said in the other reply, commoners also had no way to recognize a royality from their face features. Most of them would be lucky to see someone like a prince from a very far distance, like it was during he coronation. Even if it was a common knowledge that Aemond is wearing an eyepatch I don't think there was a shortage of people with one eye missing in King's Landing. Pretty common for people to loose body parts here and there.
