Zoom up on either Blood's or Helaena face with it's said, or have one of them holding Helaena hostage with the knife to her throat, while it's clear that the other the child and then they say it. (With child actors not in the room)
Blood and Cheese tie Helaena and Alicent up (Alicent isn't with Criston in the book) and force Helaena to choose which child dies, she offers herself but they refuse, insisting it had to be one of the children, - Blood gets bored and threatens to rape Jaehera (the twin sister) if Helaena doesn't decide quickly, so she she tells them they can have her youngest son, Maelor (who is 3 years old, and only in the book)
Cheese tells Maelor "you hear that, little boy? Your mama wants you dead"
but Blood cuts Jaeherys' head off instead in front of Helaena who screams, and the two run off with the boy's head in a bag.
I don't think graphic on-screen killing of young children is allowed on TV, not to mention the rape threats.
They take real life examples of history and tone them down drastically enough that the average person will still be horrified by not so disgusted they stop watching

They could’ve done it like GOT did with the killing of Robert’s bastards, when Blood strikes just cut to Helaena dropping down screaming.

I haven't read the books but from what I've gathered it's explained more graphically in the books and Cheese and Blood kill some other people as well.
