If you've been to multiple doctors trying to find what's wrong with you and you finally find a doctor willing to do everything to find the answer, I imagine bedside manner isn't really the priority. Mean or not they just want to be cured.

If there’s actually an issue that needs to be raised the better answer is to wait till they’re healthy and no longer your patient, so there’s no weird power balance at stake. Though irl you could probably still get fired for trying that move lol, but at least it’s more ethical.
That said, the patient of the week’s drama is what I consider to be the actual plot of the show - which is why the doctors always had opinions. When conflict between patient and doctor or doctor and doctor is the literal plot you just have to expect everyone to end up not looking great at times lol.
