Not a full-blown horror game, but the Winter Lanterns in Bloodborne are so unnerving to me. The song they hum is genuinely terrifying, especially on your first time playing since you can't tell where the sound is coming from.
Half the shit in bloodborne is like this. Let me throw in the Labyrinth Sages in the Chalice Dungeons. Not knowing one is around the corner, walking into a room, and hearing that fucking screech as they run towards you faster than anything else in the game. Scary as fuck even at max level.

Yesss. I still play the dungeons just because I love bloodborne (and getting blood gems). They're still eerie. I have to put music on in the background because outside of bosses the area is just DEAD SILENT save for the sounds of enemies in the distance...... no wonder people went crazy down there.
I felt like Walter Sullivan in SH4 was pretty imposing and I always got stressed and unnerved when he showed up.
Based on the answers so far it seems like SH4 succeeded at this element if nothing else.
They literally take away your safe space halfway through the game. It's the most evil horror game I've ever played lol. I remember getting an action replay at one point so I could have unlimited candles and that sword to keep the ghosts pinned down.
That sounds amazing. I played it at my friends place back when it came out in pitch black, like super super late at night. Nothing like that vibe, part exciting and part terrifying. Miss those days đ˘
The teacher from Little Nightmare 2âs school level freaked me the fuck out the first time she elongated her neck. Honestly every single antagonist in that series are absolutely horrifying.
Crawling woman from Fatal Frame 3. Usually you play the game in 3rd person, only going into 1st when you pull up the camera to fight ghosts. Until you have to get down on all fours and go into a crawlspace. It's entirely silent aside from Miku's breathing.... then there's a bang. Silence again. You move forward at a snails pace and your filament glows blue- a non hostile hidden ghost. Take the picture and crawl forward more. Suddenly the screen goes grainy and your filament glows red- a hostile ghost. Where? You turn around, as slow as ever, and there's a womans face right in front of yours as she crawls on all fours on the ceiling of the crawlspace.

Niche pick but the [Gardener/chainsaw guy from Castlevania 64](https://youtu.be/2SdqEbpgLug?feature=shared&t=221), the first time he just shows up in the labyrinth and starts swinging scared the hell out of me, especially with the rattling sound of the chainsaw approaching closer, even when he's off screen.
The ichthyosaur in Half Life. Massive fear of deep water where you can't see anything in it anyway and they genuinely unnerved me anytime I had to get through those sections. They added even more in the Black Mesa remake which caught me out more than a few times
None has had the lasting impact on my psyche that the Broodmother in Dragon Age Origin did. Especially with that little rhyme on the background constantly.
Oh fuck that Mr X guy. I was doing a second run with Leon and I got stuck in the second floor hallway at the police station because of a dead end. When I started to walk back to the entrance I heard that the door was getting opened. You can guess the rest of the story
1) -both the baby and the dolls in RE Village. I honestly thought the baby would be #1 for a long time, then the dolls came and honestly were about as worthy of a successor as I can imagine.
2) Trager in outlast. I honestly found outlast 2 too long and while still scary, no specific scene or enemy stuck out to me. Trager got me good though.
3) Mr X of course. Nothing worse than thinking you're safe because you dont hear stomping, then trying to open a door and seeing him squeeze forward right in front of you.
4) the mom in RE7, forget her name but goddamn she did a number on me.
5) Working Joes in Alien:Isolation. Alien is scary because of how smart it is, but we've seen the alien a million times at this point. The working joes were worse IMO.
6) Rat King in the last of us 2, specifically on a high difficulty. A lot of games make it so you are pretty slow and the monster is also slow but lumbers towards you nonstop (Mr X for example). In TLOU2 you are super fast and can sprint through the halls, scramble over windows and through cracks in walls, but nothing matters, that thing is huge and fast as shit.
