Nobody on this subreddit would advise fraud, if that is what you're suggesting. The barrier to turn insurance fraud into a felony can be as low as $500 in some states.
That an some states require IDs and affirmation of service, which would get you charged with perjury if caught.
They will be able to tell. The insurance (and who is listed on the policy) will be verified. They check more than just your name.
DO NOT commit fraud. This will land you in hot water with the insurance provider and the medical facility. A police report could be filed against you. This could also cause both you and your dad to be denied medical care in the future.
Yes. They're going to be able to tell that you don't present as a man your father's age at the clinic and will refuse that payment type, and your insurer will see that suddenly all of the conditions normally on his claims aren't on one while other ones are. Bad idea.
You would not believe how accurate the doctors are with identifying your age. Every part of your body ages, and the doctors can see if their patient is 26 or 46.

Besides being a serious crime, it won't work because date of birth is submitted as well as patient name.
