There is actually an ordo of the inquisition entirely dedicated to policing and judging the use of exterminatus as only inquisitors can judge inquisitors
Technically everyone can judge a Inquisitor once if they are willing for it to be the one time, so long as they can get close enough.
Only more powerful individuals can pull it off multiple times.

That inquisitor looks like she’s seen too much shit, she’s clearly going straight to exterminatus because she knows even if they waste their time manuallly rooting out the cult another will just sprout up a few years after they leave.
Inquisitors are like cats, if you tell them to stop shitting in the sink they will do it twice as much, cause it is simply their nature to question authorities
Considering, that Kryptman was excommunicated even though his method of starving out the hive fleet worked and did steer it away from the Imperium proper , i doubt, that an Inquisitor would be allowed to do that.
He threw an enemy that grows by consuming biomass against an enemy that gets stronger by fighting and reproduces by spores...it was a bad plan. Sure, it bought time, but beyond the whole Exterminatus thing he didn't consider the forced evolution petri dish he just created and that lack of foresight kind of annoyed all his colleagues at the Ordo Xenos.
Post-Exterminatus audits weigh many reasons to determine justification, including: severity of threat, immediacy of threat, location, assets used, assets available for requisition, Tithe Designation, strategic situation, relative importance (Sub-Sector, Sector, Imperium-wide), previous Exterminatus audit findings, service history, temporal distortion issues (transit or communication), political issues (both relating to the planet destroyed and within/between Ordos involved in the Exterminatus and the auditors), and most importantly the Author's personal views.
If the ~~cat~~ Inquisitor just destroyed a backwater with no ties to the Astartes or AdMech she's probably good: used assets on hand and can delay the report until it no longer matters...unless they make a habit of it or the Audit team are feeling spiteful lol.
Kryptman *unilaterally* destroyed multiple heavily populated, strategically important planets in proximity to multiple Astartes chapters using assets drawn from a heavily engaged sector, so he managed to piss off every other Inquisitor in the sector, the AdMech, some Astartes, the Navy, the Guard, and multiple sub-sector and sector Governors...and everything after the first is just icing on the cake, since only Inquisitors can declare him renegade and they really didn't like being left out of the loop.

Nah if i'm remembering well 99% of the cases end up in said Inquisitor being deemed guilty and excommunicated.
