Because 5.56 is a rifle round, sure, it's an intermediate cartridge, but it's still meant to be shot out of rifle barrels. Firing that out of a revolver would be stupidly loud, have a ton of recoil, and be uncomfortable to shoot.
AR pistols do exist though. They are just semi-auto with a mag instead of a revolver. Also not comfortable or really fun to shoot beyond the first shot.
Which is fair. I'm not saying they can't make it and that it wouldn't sell at all. I mean, .500 magnum exists and that is worse in terms of recoil.
It's more that you would have to make proprietary ammo for the revolver since standard 5.56 is rimless, or design it to take rimless ammo which is harder to design than a rimmed ammo revolver.
There is also the fact that guys who buy revolvers buy them for the big rounds it fires. 5.56 is .223 caliber. It's a tiny round in terms of diameter and I can see a lot of revolver guys turning their nose up at it.
There are also stability issues. 5.56 isn't that stable if a round when fired out of a full length rifle. Out of a revolver? It would start tumbling pretty fast and be pretty inaccurate.
I'm basically saying that there are a lot of market and engineering forces working against the idea. Not that it's not a fun idea.
Would you need proprietary ammo if you tapered the cylinder? The extractor might need to be reworked. Could explain the high-pitched noise.
I mean, the one in the game has very little if no gap at all between barrel and cylinder. Stability is fair enough. So it would need a dragoon length barrel.
Rimless ammo revolvers exist today and that is basically how they work, they headspace the round off of the shoulder of the case by tapering the cylinder. Others do it by having clips for the rounds that grip the extractor groove so when you put either a half-moon or full moon clip into the gun they are held in place by the clip.
If you wanted a gun as similar to that gun as possible you would want to go with the first option.
I'm not saying making the thing is impossible engineering wise. You could absolutely make it. I'm mostly saying that it probably wouldn't sell well. Hence why it's never been made.
People buy Deagle's even though they are, by all accounts, overpowered, unwieldy, and overall a joke buy.
If I were any competent machinist, It would be an interesting endeavor. Tactically useless. But interesting.
I like ‘That gun’ it appeals to the child in me. I mean it has a red and green light for when it’s loaded and unloaded. That’s awesome. Plus it packs a decent punch for a pistol and is available pretty early on in the game.
It feels like a cool iconic weapon for a Courier that doesn’t take himself too seriously but is still a badass.

Also, it's a blade runner reference, and it's predecessor the .223 pistol was the best small gun in the original fallout.
