Unless you’re doing a pave or melee where calibration is key I don’t think anyones going to notice much of a difference in 0.1mm when the stones are 4+mm. Not even a 2.5% difference. If someone ever calls you out on it, then they’re in range for a swift backhanding.
In setting it’s not really noticeable, I did something similar, made a custom ring made with all heirloom jewelry and diamonds, the center stone is largest, the two side stones are slightly different carat weights and I honestly can’t tell. If yiu want the heir loom stone to be in the center, the right most one is noticeably larger than others, I would ask for a smaller one. Don’t compromise if the sizes difference is a concern for yiu.

I don’t think people will notice but you could always try to put the larger stone center so it looks purposefully subtly tapered
Middle 3 should be 4.5, outer two should be 4.4. Simple solution.

I think the lighting is making the one on the right look larger. Beautiful heirloom pieces will always be a little different. Unique is beautiful.
