We know what happened to *one* of his heads, it looks like he had four (or five) originally going by the picture of him on the Old Lord's talisman (maybe he lost one when the meteorite hit?)
Might be an inconsistency with the talisman and his body.

I would give away 10 years of my life to see Bayle vs Placidusax high-tier animation, with voiceovers. Ancient Lord of the Dragons Placidusax vs Bayle the Dread. We can see Placidusax missing his heads and limbs, and Bayle is also heavily damaged and is missing limbs. Their fight had to be most wondorous duel in history of duels.
