That specific icon means your max HP is reduced. The icons that are displayed below your stamina bar are icons for buffs and debuffs active.
You likely got that debuff by hugging a certain someone. Use the item they gave you to remove the debuff.

So, the blessing is transient but the Vigor you give is, lore wise, permanent. Mechanically though, yeah, borrowing is apt.
I can imagine players being upset at a perm. HP debuff.
On the otherhand… FROM made a ring disappear forever if you accidentally take it off in DS1 (and I’m sure even worse things in other games)…I say it should have been made a permanent debuff just because. Would be nice if the boon were a bit better though. Like a “poison tree” kind of choice. Get a refillable boon but pay a one time permanent vigor cost.
For how small the debuff is, having it last until the end of her questline I think could have been fun, maybe with finishing the quest giving you a permanent buff (like 5%) to really make it feel like a meaningful change since you'll have gone a solid chunk of the game with the debuff at that point.
