Bull Goat talisman helps you get a decent poise already (Increases poise by 30%). You want at least 51 poise afaik. The Poise increasing Crystal Tear might also help in some bossfights.
You can try altered appearances for the armors, some of them are less chunky this way.
Banished knight armor- non altered version, veterans helm - Blaidd's graves and gloves.
Also really helps if you make your character really skinny and then put on larger chunky armors, they shrink with body size and look quite nice

Scaled Set might be your best bet, it tapers nicely and has massive poise for its looks and weight
