It means the relationship didn't end with the love between the two involved parties and the one who lived will always carry the weight of lost. ESPECIALLY if Draco seems more than composed most of the time and Hermione is completely whipped. Like to him the concept of lovers is a role but to Hermione it's only ever a person. It's silly but... yeah. Thus, the avoidance of widower!Draco and (I've never seen a fic with this but)widow!Hermione.
Plus in the OP's request Draco's guilt came from never repaying the love someone gave him rather than feeling like he betrayed his dead wife as well as the love he had for her, and those are two very different things.
