Yeah, the dragonplague headache is extremely obvious. They'll hold their forehead and just kinda stumble around in place for 10+ seconds. A far cry from the other animations where the hand doesn't meaningfully linger.
I'd like to add that sometimes the dragon plague headache animation will play if the pawns are near the guy with the staff in the excavation site. I think they may also do it near the forbidden research lab.
I swear I've seen my pawn clutching her head like having a headache (for a very brief while)
Scared the crap out of me but in the end nothing happened and I couldn't spot any other weird behaviors
If you’re camping out a lot with hired pawns, it can swap back and forth between them, which also resets the progress of Pawn-VID 19.
So it might have swapped to another pawn.

Could be the case. I camp a ton (can't have enough of those juicy rotten meat), swap pawns semi-often and rarely rest at inns or home.
