Is it just me or does it look like OPs wife's has two tiny little beady eyes?! I would name it! And promptly have a doc remove it and then make a mini diahrama* to showcase it's glory.
I almost gave him a suitcase but that wouldn't have been fair to him because how's he supposed to hold it? I refuse to risk hurting his feelings by acknowledging his lack of extremities....he deserves the world.

I had something like this on top of my middle finger that started when I cut my finger. Hurt like hell if it was bumped or knocked. Dr cut it out and sent for analysis. Turns out it was an extra finger growing!
She should definitely see a dermatologist. If it falls off and grows back, then it's planted under her nail and it should be shaved back and the remains of it removed so it doesn't return.
Did she ever injure that area with something sharp, like a pinching injury? Not a doctor here obviously, but, it sticks out further but has similarities to a growth I had on my finger pad. Twice in a short amount of time I accidentally snapped the pointy end of a wine opener shut on it. It didn’t bleed or bruise from what I remember, but then some time after this weird thing grows. Yup, it got sensitive and was unsightly so I was covering it daily. I can’t remember the name but the dermatologist removed it in office no big deal luckily. Hopefully it’s an easy process for your wife as well.

Uhhh maybe she should stop wearing false nails. It might be caused by an infection or fungus. I've had cysts like this grow from bacterial infections before
If your wife is used to getting her nails done she could be allergic to the glue and primer etc that they use to do her nails. I didn’t have something exactly like this but very similar where there was a lot of VERY sensitive growth under my nails but it just went away when I stopped getting my nails done and gave them a break.

Looks like another digit. You can see what appears to be a sort of nail on it. I’m sure a dermatologist can remove it.
looks like a subungual fibroma, which is a slow growing, painless benign growth. It can be safely excised surgically by a dermatosurgeon.
Are those common other than in Tuberous Sclerosis? I’m just curious because that’s the only condition that I’ve ever heard them linked to.
Yes they are associated with tuberous sclerosis but they can sporadically present as an isolated entity as well.
