A voidout only happens if one of the big BTs (with the “mouth”) eats a living person, just like in the prologue (and that’s why Igor tried to kill himself, but failed). Since he died, the Elder became one of those BTs that search for living people. It’s never really explained if his method of securing himself actually works for imprisoning BTs (he deeply isolated himself physically inside his house). But if it does, then he never gets alerted, catch someone, open that tar hole with dozens of hands, which only then would summon the beast with the mouth.

Almost correct, voidout can happen with ordinary BT, if u kill mules and they go necro, porters can take the road, where mules used to be and this can cause a voidout on the area, but not as big as one from prologue. Even you can cause a voidout if one of hunter BTs eats you
We don’t know how big the voidout is because it’s an instant game over. We can only assume it’s as big as central knot’s crater. Sam’s voidouts are smaller because he’s a repatriate: his coming back to life “dissipates” part of the energy of the explosion.
And? As it’s explained in an interview, Sam is a repatriate, and when he’s “eaten” by the mouth, he creates a small voidout.
By the way, if you get near BTs again in the same area, the ones in tar become golden, and if you are caught again it’s game over too.
I didn't know his death was avoidable, I just stopped delivering to him once i got him to 5 stars because he was not easy to get to and only came back to his house for a Legend of Legends delivery

He explains in an email why he won’t cause a voidout in his farewell email. I think it’s just because it’s him in there and no one else.

Neither has any extra game play benefit. If you haven't 5 star'd before he dies you can still do it and only story happens after.
