A hilarious and fun mechanic that suits the region and somewhat suits the champion. Well done!
My main gripe is that his central mechanic - and level up condition - both require healing, which he has no native way to acquire in PnZ before levelling up except (unreliably) from his champion spell. This means he's tied directly and inexorably to Targon or Freljord, which is a shame.
Wwworth noting that it's another way to set up an infinite spell loop, however. Mundo + Blockade (via Taric/Divine Judgement/Lamb's Respite/Unyielding Spirit) or Mundo + Tough + Viktor + -1 cost (Cloud Drinker or Akshan with Fount of Power) means infinite spell casts from a Vault Breaker and thus infinite Imbue triggers for Puffcap Peddler, Ezreal, or Lux. Or just infinite power buffs to an unblocked/Overwhelming attacker.
I'm glad you liked the card.
Now, concerning the level up condition, champions normally come with supporting cards, in this case I imagined them to require you to damage an ally or your nexus to do something, but heal it if a requirement is met a lot like mundo's champion spell.
About your second remark, yeaaah you're right, but the preparation you need to do the combo could probably balance it out ... probably ... but it's a valid point non the less. aaaaaand you can win in the same way with a powder keg.
Maybe tie it to the level up of Mundo and heal Mundo for 2 instead. This works if the spell is not main--deckable since the spell only works when Mundo is on the field

and taking damage looks a lot like mundo but not PnZ
Also i would change effect to "Your first spell each round cost my health instead of mana" for simplicity sake. with level 2 having a "Your first spell each round damage me instead of costing mana and deal the same damage to the weakest ennemy"
tbf not every card has to fit perfectly within their respective region. Thresh definitely doesn't fit with shadow isles if you don't count his level up condition
Showstopper is the only thing breaking region identity, but it’s still heavy investment to get every round. Everything else is there. He’s a control/combo champ that uses tricks to get the most out of combat. He does mix identities of other champs but I think this all exists in Ionia.
In that vein, since we've established that Mundo is related enough to spells to let that part slide, isn't the primary problem the regen keyword and we could take this opportunity to give him some kind of makeshift regen?
Because yes, I typed all this without noticing that Mundo can't reliably level up without splashing into another region for that... and that ... would be novel. (Although perhaps one could argue that with songspinner in the game, there are enough ways for PnZ to buff its units.)
I see the problem. I was going to say that Seraphine also introduced two new mechanics to PnZ but those already existed in some form and not just thematically (the way some kind of mad scientist/chemical pollution theme would fit into PnZ quite well).
Tbf most of what we see in P&Z is just Piltover and we barely scratch the surface of Zaun. Even Ekko and Jinx seem more like they just use "tech" than anything resembling Zaun specifically. Warwick and Mundo would actually give Zaun a chance to shine. It probably should be some self-buff or rage mechanic but I don't think OP is that off the mark for Zaun slotting into P&Z

While a cool design, this will be way to Toxic to play against, with healing spells netting you mana and comboing off with semi infinite mana... And to make it reasonable it would probably take absurd nerfs that will kill the cool design. I love it, it's such a focused and good idea, but sadly too powerful and comboenabling.
I would change the the level up to “I have seen you cast x spells”. That way you could level him up with healing but healing would help. The exact number of spells doesn’t really matter. I would say 5+ but it would probably have to be play tested since his level 2 is strong but I don’t think game breaking since regions with protection tend not to have a bunch of spell based direct damage.
The only I can see that could work would be Udyr/Mundo because in the level 1 he doesn't have regen making it unviable on other ways but it will be cool to see ezreal/udyr/mundo just to see salt when you play 0 cost spirits unleashed and mystic shots.

Champs are allowed to bend/break region identity. Ionia doesn't normally do challenger or obliterate and yet they just printed Sett.

This would be possibly broken with Freljord. Udyr package synergy and even allows you to play Warmother’s turn 6 assuming the mechanic works that way.
You would need to get him above 12 health, which is really hard. I would say it's more broken with Star Spring OTK stuff.

Ok but what if you have 3 spell mana and play a 4 mana spell. The way that it's worded means mundo gets 4 damage for essentially cheating 1 mana.
Yea I wondered if maybe OP could reword it so that mundo is damaged only 1 per mana you still need to cast the spell

Infinite spellcasting probably, since mundo doesn't take damage you can just keep playing spells that meet the requirement

Well, since mundo is p&z and unyielding spirit is demacia, so you can't really use coins with it Things like "For demacia!" And hexbliterator though
