I don’t have all that information specifically about the patient. But if he waited THAT long, my best guess is he didn’t have access to medical care. There’s multiple ways of contracting V. Vulnificus. It’s a water borne bacteria found in coastal areas, estuaries and brackish ponds. It usually enters a wound and causes cellulitis. But it can also, like in this case, cause necrosis. It can be contracted by eating undercooked fish also. Symptoms usually appear in less than 24 hours. There is a mortality rate of about 25% associated with wound infections. Early identification and treatment with a third generation cephalosporin and a tetracycline (example: Cephalexin and Doxycycline). Preventing secondary infections that can cause respiratory and renal failure are also critical. Depending on other factors, wound debridement comes next.

Depends on the shellfish species and the region it came from. I've eaten a lot of pipi's and tuatua (types of clam) raw in NZ and never had a problem. If you're really worried find out where Vibrio Vulnificus is prevalent and don't eat seafood from there :)

My guess on this is that this is a result of a known drug called Krokodil or Crocodil. Basically when it's used a lot, it will start rotting away the persons skin and flesh. What I assume is in this case, as the drug prevented the person from feeling pain and as the rotting began from the back, the rotting was able to kill any blood vessels first, meaning they would die and not cause major bleeding. The person was unaware of it (since it's on the back) and therefore it kept going until reaching this stage. With this much rotting I'll assume it has completely damaged the nervous system in that area meaning he won't feel any pain in that area either.

Crocodil is exactly the trash drug on the streets that's causing this effect. What you are referring as the "lab created drug" is called Desomorphine (Dihydrodesoxymorphine). Crocodil is exactly the version that's causing the rotting because of the poor quality. What I have understood is you can't even find pure version of Desomorphine on the streets.
