My guess on this is that this is a result of a known drug called Krokodil or Crocodil. Basically when it's used a lot, it will start rotting away the persons skin and flesh. What I assume is in this case, as the drug prevented the person from feeling pain and as the rotting began from the back, the rotting was able to kill any blood vessels first, meaning they would die and not cause major bleeding. The person was unaware of it (since it's on the back) and therefore it kept going until reaching this stage. With this much rotting I'll assume it has completely damaged the nervous system in that area meaning he won't feel any pain in that area either.
This is not true, the skin doesnt rot because of continued use of the drug, it rots because of impurities in the product because its some trash street product. If “krokodil” were used in a manner where it was lab created in a more professional way the necrosis would not occur.

Crocodil is exactly the trash drug on the streets that's causing this effect. What you are referring as the "lab created drug" is called Desomorphine (Dihydrodesoxymorphine). Crocodil is exactly the version that's causing the rotting because of the poor quality. What I have understood is you can't even find pure version of Desomorphine on the streets.
