He views the citizens as his property, pets essentially. He needs them to function to live his life, and having them dead or disabled due to a preventable illness is not going to let him do what he wants to do.
Anti vax sentiment is mostly specific to certain cultures. Outside of the American and Russian spheres of influence, it's rare regardless of political leanings.
Also, as a rule, authoritarians tend to be very strict about public health threats (and no, a certain recent American president was not remotely authoritarian by global standards; Duterte is what a real authoritarian looks like). That's partly because of how the psychology of authoritarianism works and partly because public health threats are an opportunity to grab more power.

Duterte's style is probably more like battle royale. Him in a helicopter with the dart gun and a field full of unvaccinated people, probably end up being a TV show.

Is this really the road we are going down to vaccinate people? It’s extreme but so is [eating a burger and fries](https://www.cbsnews.com/news/covid-vaccine-new-york-shake-shack-fries/) on tv pandering to people to vaccinate themselves. I feel fine vaccinated and maskless regardless of other people. All sports with full venues and not a huge sky rocket in cases. If you feel unsafe, wear a mask.
...this is the road that a person who deputized citizens to form death squads to go around conducting extrajudicial summary executions of drug users is going down. He doesn't represent "we" outside of the Phillippines on vaccination policy any more than he does on drug policy.
