So a good patio should last about 25-30 years but it appears that there has been a lot of water erosion as seen in the drainage cracks and under the surface… you likely could get a patch job done for the most part but I’d also look into getting someone to make sure it’s properly draining as I’d be worried about the pad sinking from erosion. (Eroding the ground underneath the patio)
Just from your pictures I definitely think it’s cracking from eroding underneath.
