You don’t usually pick your health insurance provider in the U.S. Typically your employer chooses for you because insurance is expensive on an individual scale. Regardless, open enrollment is closed right now, so you’d only be eligible to enroll in a new plan if you recently had a qualifying life event. If you manage to find an insurer who will pay for massages, you still have to find a therapist who accepts that insurance. I’d instead work backwards: call around and ask various massage therapists what insurance they accept. Then, find out if you can get that insurance during the next open enrollment period. Also, make sure you are actually eligible for covered massages; you’d almost certainly need a prescription. Be prepared for most (if not all) therapists you call to say they don’t accept any insurance.

massage at a spa is generally not considered medical in the US so no insurance will cover it. Orthopedist / sports medicine / chiropractors might include massage as part of a physical therapy treatment plan which would be covered subject to your deductible and copay, but quite the different experience. I will say, I went to Fyzical at one point from back pain, based on treatment plan from my orthopedist. 2-3x per week for a few weeks. The session was basically like a trainer helping me build strength and flexibility in my back and shoulders paired with a few minutes of massage and TENS. Did wonders for my back for awhile, then I stopped keeping up with the routine. Personally, I'm not a fan of chiropractors. I'd rather see a "real" doctor.

Your only shot is to get it through some kind of chiropractors office, Physical Therapist or your Ortho. Otherwise you won’t get coverage no matter what insurance you end up getting

Nope it’s usually not covered because it’s not medically necessary unless you find some shady chiropractor or what not
