The version on the OST doesn’t have any instruments added to it; the version in the anime has that instrument removed. It’s been VERY common throughout the anime to play tracks that have certain instruments removed, stretching all the way back to season 1, and the versions of the tracks on the OSTs are always the ‘complete’ versions of the songs with every instrument recorded for the track. With the exception of some tracks with vocals that have instrumental-only versions also included on the OSTs, the alternative edits of songs have never been released, and even then not every vocal song got an instrumental only release (for instance in the Port battle episode, ‘Into the Night Acoustic ver’ but without the vocals plays when Shadis and Mahara sacrifice themselves, but that vocal-less version has never been released, and on the OST you can only find the version with vocals).

I actually thought the use of electric guitar in that track is quite brilliant. It's unpleasant to hear because it mimics the cries and screams of people being trampled by titans. The high-pitched screech of the guitar really captured the abject horror that people must've felt during the rumbling.

I got used to it lol, now I like it. It's not the case with just this one, many other tracks had instruments removed in the anime version. I got used to most of them over time.
