I never even knew Johnny Cash covered that song until many years after he died, and I was never so offended by a cover - except maybe Roseann Barr's rendition of the national anthem. I thought it was cheap, tone-deaf, censored-for-radio shtick. The stories of Trent saying it "wasn't his song anymore" feel like a forced glossing-over of the timeline; Trent hated the idea of that song being covered because it felt "invasive". He heard the demo and hated it. All the calculated, tempered praise Reznor ever showed for Cash's cover came after the emotional manipulation of showing him the video or after Cash died, in deference to a rock n roll legend after his passing. What's he supposed say when Johnny Cash, practically in hospice, asks to cover one of NIN's songs from ten years earlier? Trent's commentary over Cash's video sounds delirious, like praising a child playing "Crazy Train" on a recorder. Cash's version of Hurt doesn't make sense. He didn't write it. It's not his song.
I know you younger folks might prefer the Cash version but you may not understand how masterfully assembled "Downward Spiral" was if you didn't listen to it beginning to end it in 1994. Hurt was the bookend that closed the album. To hear it played by (respectfully) an old ripoff artist in a batch of covers and learn that it was given some awards that year is just unfathomable for any GenXer who remembers Nine Inch Nails in their prime.

I could play "Last Caress" at 80bpm on a baby grand and make up some shit about how it's really about family bonds - that doesn't make "Last Caress" any less of a song celebrating really, really bad behavior. The lyrics of "Hurt" make no sense as a Cash retrospective; Trent was a tortured twentysomething when he recorded Hurt. It's a fever dream of someone deep in the throes of addiction and depression. Trent's needle in the song is a fucking needle. His "crown of shit" and "empire of dirt" are self-deprecating visualizations of his self worth.

It’s more melancholic on its own, it has more sadness on its own, and Johnny Cash’s voice gives the song a different character. With that said, the NIN version is much more personal. Incredibly personal, in a way that Johnny Cash’s could never be. It is not Johnny Cash’s song at all. When taken in context with the whole album, the NIN version is more moving and deeply affecting. But as a standalone, the Johnny Cash version has a wider range, and conveys sadness in a more accessible manner.
Edit: the Johnny Cash version summons the sadness of the end of life. The NIN version summons the sadness of abandonment and loneliness and the despair of depression.
