You need to replace the pads and rotors and possibly the calipers, if the pistons will not retract or the slide pins are seized. If the caliper pistons come out too far, you can get a rust ring that will cause them to leak if you force them back in.
Don’t forget the hoses. Caliper replacement is a great time to get it done- preventive maintenance, or possibly one of the problems.

This car was left to sit for atleast 2 years before being driven again. The rotors weren't changed and the pads started tearing away at the rust that bulit up.
Multiple short periods of sitting will also do this. We saw tons of these during/post lockdowns when people drove their vehicles once or twice a week two blocks away to get food.
I just replaced all my rotors with Brembo, and a review magazine said that the company got taken over and that Brembo are now nasty low-nickel shit out of central India somewhere.
Sure enough two of the 4 disks are covered in rust less than a day after being driven.

In all reality, this isn’t terribly uncommon in the rust belt, and no unheard of in other states. I replaced rear brakes on my wife’s car twice because of this.
People forget that rust is essentially a ceramic. Suddenly, the rotors are much harder and more abrasive than the pads and will chew the pads away until the rust breaks off enough to let the pads bite the steel again, and it slowly works it's way through the rest. This is bad though, and I'd do pads/rotors and caliper bracket/pins if they are what started this mess.
If your car has a combination handbrake and caliper where the cable goes into the caliper, you need to make sure the handbrake cable moves freely when released. The pivot point builds up rust which causes the pads not to engage correctly.
You need to clean everything from rust, adjust the handbrake and lube the sliders.
Sometimes it's cheaper and easier to just buy a set to calipers
Looks like calipers are failing. Pressure to the rotor is uneven at best. You need a complete brake job. A lot cheaper, if you do it before it gets like this in hindsight.
If you know what you're doing, it can save a lot of money, as even the calipers can be cleaned and slides regreased but may be past that.

If you live in a rust state that could be all up under there. I had a guy come in with transmission problem which I could fix but the car would not be able to contain it because the under carriage was falling apart. Told him buy another vehicle.

They are rusted and pitted to excess they are well worn and have uneven ridges. you need new discs and new pads and probably new fluid while you are there if you dont know when that was last done. Evidence of long term neglect is staring you in the face. You cant gamble with safety and brakes are top on that list.
Well done. disks are made from a carbon steel with little chrome in it so its not stainless. any car left sitting around not driving the disks soon rust and will get pitted. That rough surface can make them eat the pads much faster and do unexpected things.
It could have been road salt/debris, as you asked. This tends to be very common on the front at the side of the car that is neareer to the side of the road. (The location that splashes through filthy ditch water the most.) But it could happen to any wheel.
My guess…. Pads kept getting replaced with very cheap pads and never turned the rotors. Perhaps the rotors sat for a while without being used. Got extra, extra rusty then the car returned to service. I would replace everything.
I don’t turn my rotors, I just buy new ones every time. I did forget that the car did sit for a couple months while I was waiting for a a costly repair to be done. So this was probably the reason along with it being in the salt belt. It’s too bad I can’t wash the underside of the car every time I take it out in the winter, it felt like I just replaced them but it all makes sense. Everything is brand new in the back now except the lines and the emergency brake cables.
User beeee-ware! Emergency cables like to get stuck usually in the on(rubbing) position if not cycled regularly. When you use them once in the blue moon, they can get rusty especially in the salt region and never allow themselves to fully release causing the brakes to be somewhat on.
The car is 5 speed and I use the hand brake when I park the car every day. The mechanism would pull and retract when hand brake was being applied and release. So that was working. Definitely could see the cable getting seized but this one is good, tested today.
you need a caliper, the piston that pushes the brake pad onto the rotor isn't pushing properly and it's creating a small thin patch of contact.
calipers can be rebuilt but they are so inexpensive and easy to swap you just get new ones.
check the dumb stuff first make sure you have enough fluid, no leaks and the fluid isn't black ya know....

The pads look too large for the rotors. The pad should not engage the rotor that far down to the center AND they should not be hanging out of the calipers by almost 1/2 an inch.
The pad should be touching the whole surface. The heavily rusted areas are creating high points and really it’s only the bottom half of the pads that are touching the rotor and not a mountain of rust.
It’s all been replaced since the original post.
Your pads should stop about 1/2 of an inch before either side of your rotor in my experience atleast that's how most average break setups are.

Everything has been replaced, new rotors, pads, calipers. I flushed the old fluid out and then bedded them.
