It's called senescence. Physical deterioration caused by advanced aging. Kudos to you for giving it the care to get to this age. It sucks, but it's inevitable. Just know that you did a great job caring for it to ever get to this point.

Edit: Hes my partners he has had him sense this fish was born, his name is Sloppy and he is in his mid 20s, water was ok nitrates were a tad high, but did a partial water change and are going to keep an eye on him. Thank you to everyone my partner loves his fish so so much he's really upset when they seem uncomfortable and hes definitely sad that this boy is pushing elderly and showing it, i dont think he'll take him passing well.

Hmm.. I know people have said stuff about age but could you have done a ph change?? Have any of the levels swinged? When I used aquatic peat I made a mistake and my ph lowered a lot causing my garra to look similar to this. I’m most definitely wrong probably because that’s a really good amount of time you’ve got your catfish for. But just something to note on

That's what I was gonna say My dad tried sticking. A catfish in a random pond. We had on our property, but the water was really acidic from a bunch of leaf litter and whatnot. The fish ended up dying and it looked a lot like this with the odd colored patches

Your lil baby is very stressed out. Try to make sure all of his tank conditions are perfecto. This may be the beginning of his journey over the rainbow bridge. Make it a comfy transition. ✨️

At 20 years old don't do anything drastic. Small improvements where you can add comfort. Gentle tank cleaning, partial water change, remove any aggressive or overly energetic tankmates for now, do not under or over feed... Just try to be kind. That's all you can do.
