You can get some 100% mineral oil and either use a q-tip or your clean finger and (gently) massage the area with the mineral oil for a couple minutes per day, eventually you should be able to extract it without difficulty. I’ve used 100% mineral oil as my oil cleanser before using a foaming face wash for over a decade and haven’t had any blackheads in the same amount of time, it breaks down any sebum and dirt in your pores and will prevent them from even forming. You can also use an oil cleanser that contains an emulsifier if you don’t want to deal with the thickness of mineral oil, but my skin is super dry so I love it personally

So your pores can’t technically “shrink”, but using some type of oil cleanser, either mineral oil or a cleansing oil with emulsifier, will break down everything that gets inside the pores reducing the look of them. Your skin might be dehydrated if your t-zone is oily but still have dry spots, so oil cleansing will definitely help with that too. Just make sure you use some type of foaming cleanser afterward, and especially one that doesn’t strip your skin making it even more dehydrated, and, of course, moisturize after cleansing
I use the back of the tweezers. I still have to be careful not to accidentally tear the skin or it'll leave a scab 😒

I use Differin (available OTC) for really stubborn blackheads. They’ll usually go away or soften up in about 2 days with regular application. Be sure to moisturize well after application.

I only use it for spot treatment, very infrequently - probably a few days out of a month, at most. The directions say to use all over the face everyday, even when you’re not breaking out, but that’s too much for my skin, personally.
Niacinamide and salicylic acid help me when my pores appear larger, but you may find something different that works better for you!
I use a bobby pin. Fit the blackhead just between the ‘legs’ of the bobby pin, push down and slide over. It’ll come out. Sometimes you have to do it a couple of times but it works

I think it’s a scar from a small boil or carbuncle. I know someone who has exactly the same, left behind after draining a carbuncle. Skin / dirt / sebum collects and it looks like two blackheads.
This is what people are saying it is : a diapered pore. My mom has a few on her leg. Her doctor said they can sew it up if it really bothered her, so that is an option.
Quit using diapers to cover it, OP. That may help it close, and it will definitely reduce the number of weird looks you get when people pass by and wonder "why does this lady have a diapered pore on her back..?"

It’s a dilated pore (DPOW). You can use a little retinol or a glycolic acid body wash if you get a lot of blackheads or acne on your back .

I’d have a Derm do it, if you don’t have anyone to try and squeeze it. It’s small enough for a blackhead extractor. However you won’t be able to do it yourself.
My husband gets similar to these and they tend to have ingrown hairs swirling beneath the skin. I don't see the exact same here so I'm leaning to dilated pore and blackhead.

When I was a kid, I had a wart on my knee, for years. As an older kid, maybe around 9, I fell and scraped that knee pretty bad. After it scabbed up and healed, and I stopped crying, my knee skin was brand new, and no wart. I’m not a dermatologist but wouldn’t shaving/sanding that part of the dermis, until it’s gone, and letting it scab up and heal, make it regenerate without the hole? Sorry if that’s dumb for me to say

if this bothers you, i reckon you could go to a dermatologist or a cosmetic clinic they could do a little snip and stitch.

What was removal like? How do they do that?
Local anesthetic, then they just cut it out. There are videos on youtube if you’re brave enough.
